Highway and Public Works (“HPW”) eServices has purchased Adobe Experience Manager Forms – Cloud Service and Adobe Sign Solutions (collectively, "the Adobe platform") to help fulfil its responsibilities for authoring, designing and publishing Yukon Government (“YG”) online forms in collaboration with YG Departments, including the Department of Health and Social Services (“HSS”).
eServices has engaged a vendor, Ensemble, to implement the Adobe platform across YG. This platform is hosted on Adobe's cloud. The storage of personal information collected through the Adobe platform on behalf of the YG Departments, will be discussed in further detail below.
eServices will work with other YG Departments to design the online forms required for those Departments’ programs and activities.
The Adobe platform allows eServices to create online forms and configure the workflows for these forms as required. These workflows include how the information collected via these online forms are shared with each YG Department. In some cases, this may be configured to automatically share form submissions via Microsoft SharePoint or a YG Database. In other cases, this will be configured to automatically share form submissions via email attachment (i.e., PDF or CSV). This will be discussed in further detail below.
While the overall Adobe platform will be managed and supported by eServices, the Personal Information (“PI”), or Personal Health Information (“PHI”) that may be collected via the forms will be determined and approved by the Department. This includes other data management and privacy considerations such as consent for the collection, limiting collection, and any use and/or onward disclosure of the information. As will be detailed below, these considerations are not within the scope of this PIA but will be included within the privacy assessments conducted by each Department.
