The Yukon Department of Health and Social Services (“HSS”), Insured Health Services (“IHS”) division has released a new Yukon Health Care Insurance Plan (“YHCIP”) card for eligible Yukoners. This new card replaces paper health care cards with engraved plastic cards that have added security features: anti-scan patterns, multiple laser-receptive layers, and unique document control. The plastic cards are direct mailed to Yukoners.
To ensure these cards can be issued to Yukoners with their correct and current information, Yukoners can currently update their personal health information (“PHI”) with IHS via phone or in person. To enhance the client experience and provide Yukoners with an additional method of updating their PHI, IHS has worked with Yukon Highway and Public Works (“HPW”) eServices branch to create an online form (“e-Form”) that Yukoners can use to request an update of their YHCIP card information. Per their engagement with IHS, eServices has built the e-Form in Adobe Experience Manager (“Adobe”). The privacy considerations for developing e-Forms in Adobe have been considered in a separate PIA completed by HPW (Adobe Experience Manager and Adobe Sign PIA, 2026 see Appendix K).
