The Long-term Care (LTC) Unit of Continuing Care (CC) is implementing colour-coded
wristbands to assist with out of territory evacuations of residents to other care facilities during
emergency situations.
This project has been initiated to identify residents and support enhanced resident safety in
times of crisis to quickly communicate essential information, such as allergies or special medical
conditions of patients by allowing healthcare providers to easily identify critical information
without access to the Electronic Health Record (EHR)
Relevant information for the initiative will be extracted from the EHR (Gold Care) to populate
the wrist bands via a special printer attachment. The wrist band will support transitions in care
and align with the safety requirements around two resident identification factors, supported by
Accreditation Canada (Appendix O). Using at least two person-specific identifiers may help
avoid harmful incidents such as allergic reactions, unsafe discharges, medication errors, and
wrong-person procedures.
The wristband will be used only during emergency response and evacuation activities.
