Panorama eForms

This PIA Addendum documents the implementation of the BC Provincial eForms Solution (“eForms”) by the Yukon Department of Health and Social Services’ (“HSS”). Yukon Health Service staff will use eForms to collect and use personal health information (“PHI”) about Clients in Panorama. The use of eForms aims to decrease the volume of paper forms, improve standardization, facilitate documentation and reduce the risks associated with fax and email transfers of PHI.
